The following links are to the County School Superintendent's offices in Arizona where available, or a link to the County web site for counties that do not have a School Superintendent's page. 

NOTE: Please mail your Affidavit of Intent to Homeschool to the County School Superintendent for the county in which you live.

In June 2006 we contacted all of the County School Superintendents' offices to find out whether their office requires a parent to submit the original certified birth certificate or if you can submit a photocopy of the certified birth certificate. This information is included below.
